It could happen, it could happen!
Theories abound, but no real solutions.
Hm… my next dream will be me ontop an Everest sized pile of 100$ bills. Maybe I’ll be able to come back with a few in my pocket.
Willing suspension of disbelief?
The Three Stooges poster is easy to make out. Does anyone have any information on the PvP poster? Or the one next to it?

I think it’s Art Baltazar’s “Tiny Titans”, but i’m not sure if they were around back in 2006 when this comic was made.
Dreams are rarely, if ever, in real-time.
Much like sequential art, the mind builds a story from mere fragments… short actions, images, sounds, etc… and simply imagines that it experienced the intermediary portion. That is pretty much why one can have days of dreams in the space of 10 minutes of sleep: you’re not actually dreaming it in real-time so much as you’re getting a montage, and don’t even realise it.
Similarly, objects in dreams may appear to be especially vivid examples of whatever they’re perceived to be (e.g. swords and the like), but then one isn’t imagining anything except the most basic attributes of their appearance. You might be able to describe the shape or colour of such an object in hindsight, but when you think about it you might not be able to describe its texture, its weight, its smell… etc. What you have is a grossly simplified representation of what you’re imagining… barely more than a mere symbol.
What it all comes down to is that the mind is very easily tricked. It sees the most rudimentary image of something and assumes the rest is intact. It pretty much views the outside world the same way, except the extra data provided by a physical source means that the perception of that source can always be further fleshed out.
In any case…
… Alex doesn’t have internal organs… because they’re not deemed necessary to portray him as a human being. He can swim with mermaids and fly with elves because he has exactly the same degree of realness (or lack thereof) as they do… whatever that means outside of a pseudophilosophical monologue.
